Please fill out the complete form and submit.  

Registration is 10/1/09 through 4/1/10 and space is limited.  Submission does not guarantee participation.

We invite you to join us for an appreciation dinner on Friday at the close of Ed Day!


NOTES: 

1.       ALL VENDORS MUST SUBMIT A FORM FOR APPROVAL BY THE L.M.C.W.G. . 

2.       ALL VENDORS ARE LIMITED TO 4 PEOPLE IN THE BOOTH AT A TIME AND (1) VEHICLE BEHIND THE BOOTH.  ALL OTHER VEHICLES MUST BE PARKED IN THE GENERAL PARKING AREA. 

3.       A LIST OF THE PEOPLE WORKING PER DAY IS REQUIRED PRIOR TO THE EVENT.

4.       ALL VENDORS MUST PROVIDE THEIR OWN ELECTRICITY IF NEEDED.  ALL POWER DEVICES MUST CONTAIN FIRE SUPPRESSION DEVICES AND MUFFLERS TO INHIBIT NOISE.

5.       BY SUBMISSION OF THIS FORM, THE L.M.C.W.G. IS NOT RESPONSIBLE OR LIABLE IN ANY WAY FOR ANY ISSUE RESULTING FROM YOUR PARTICIPATION IN THIS EVENT.

SET UP: 
Thursday 8:00am to 5:00pm, Friday, 3:00pm to 5:00pm and Sat/Sun 7:00am to 8:30am, with all vendor vehicles and extra set up people cleared from the event area by 8:30am.  For Sat/Sun, both days, vendors open at 9:00am.

Education Day is on Friday and option if you would like to be open.
